WHO CAN ENTER?

Entry is open to female residents of Australia aged 18 years or over  who meet the eligibility criteria for one of the Bupa Find a Healthier Family Surveys as outlined in Paragraph 3 of these Terms and Conditions (‘Entrants’). Employees of the Promoter and their immediate families and agencies associated with this promotion are ineligible to enter.
 
Eligibility criteri
Bupa Family Health Survey 1: To be eligible to complete this survey, Entrants must be an Australian female, aged 18 years or over, who is pregnant or who is planning to become pregnant in the next 18 months;
Bupa Family Health Survey 2: To be eligible to complete this survey, Entrants must be an Australian female, aged 18 years or over, who has given birth in the last 24 months; or
Bupa Family Health Survey 3: To be eligible to complete this survey, Entrants must be an Australian female, aged 18 years or over, who have a child/children between the ages of 2 and 18 years.
 
PROMOTIONAL PERIOD:
The Promotion commences at 12:00pm (AEST) on 22 August 2014 and closes at 5:00pm (AEST) on 30 September 2014 (“Promotional Period”). The draw will take place at Bauer Media Pty Ltd, Level 7, 717 Bourke St, Docklands on the 14 October 2014 at 10:00am (AEDST). The winner will be notified in writing via email and telephone using the contact details given in their entry. The winner’s name will be published in the public notices section of the Australian on 23 October 2014.
 
HOW TO ENTER:
The first (1) valid entry drawn at random from all valid entries received during the Promotional Period  will win the following prize:
AUD $10,000.00 – awarded in the form of a cheque made in favour of the winner.
TOTAL PRIZE POOL IS VALUED AT $10,000 (including GST). Prize does not include any ancillary costs associated with redeeming this prize. These are the responsibility of the winner.
