Employees (and their immediate families) of the Promoter and agencies associated with this promotion are ineligible to enter. PROMOTIONAL PERIOD: Promotion commences 09/06/2015 and entries close 11.59pm AEST on 17/07/2015. HOW TO ENTER: To enter, individuals must, during the Promotional Period, visit www.eyelines.com.au , follow the prompts to the promotion entry page, input the requested detail and submit the fully completed entry form. Individuals who have had an eye test between 09/06/2015 and 17/07/2015 automatically receive 2 tickets in the draw to win. PRIZE: There will be one winner for this promotion.
