Win a Mother's Day gift pack - National Pharmacies is offering members the chance to win one of five Mother's Day gift packs Terms and conditions: 1. Entry into this draw is deemed acceptance of and agreement to these rules. 2. Information on how to enter and the prize form part of the conditions of entry. 3. Entry into this draw is open to the public, excluding employees and their immediate families of Friendly Society Medical Association Limited and their associated agencies and companies. 4. To enter, members must fill out all components on the entry form online by midday on Wednesday 7 April, 2014. 5. Entrants will be put into the draw. Five winners will be chosen based on the best response to the question. The judges decision is final and no correspondence will be entered into. 6. Each winner will receive a Mother's Day gift pack. There are 5 packs in total. 7. Pack 1 is valued at $217.94, Pack 2 is valued at $264.85, Pack 3 is valued at $288.54, Pack 4 is valued at $292.35, Pack 5 is valued at $298.95 8. The winners will be notified by telephone on Wednesay 7 May, 2014. 9. Prize is not transferable and are not redeemable for cash. 10. The promoter is Friendly Society Medical Association Limited ABN 69 088 347 602 of Level 1, 52 Gawler Place, Adelaide South Australia 5000. 11. All entries remain the property of the promoter. 12. Any change in the value of the prize occurring between publishing date and the date the prize is claimed is not the responsibility of the promoter. 13. The winners, and the persons they nominate to take, release the promoter and other organisers, their related bodies corporate, affiliates, officers, agents and employees, from all liabilities, loss and damage of any kind arising at any time out of or in connection with the acceptance of, and participation in, the prize. 14. One entry per person. 15. The competition will start Wednesday 23 April and will go through until midday Wednesday 7 May,2014.
