(iii)   Upload a picture of your dad being a ‘Smith’ and tell us what kind of a Smith he is. Being a ‘Smith’ means being creative, innovative, or doing something you love.
(i)     Complete and submit the online entry form in accordance with its directions, including without limitation entering all information required, such as the your first and last name, mobile phone number, residential address, email address and date of birth.
