(b) locate the Eligible Purchase transaction code which can be found on the Eligible Purchase confirmation email; and
(c) follow the prompts on the Website to fully complete and submit the online entry form including by providing their full name, date of birth, email address, phone number, Eligible Purchase transaction code, and any additional information as required by the Promoter.
